Define consistent platform identifiers as described in pagePlatforms.

Most work is to cope with the variety of definitions between compilers Visual C++ v1.52 & 4-6 and Borland and Symantec on Windows.

Todo:

investigate if other carbon definitions can be used to detect _Macintosh_Carbon

automatically define _Macintosh_OSX if gcc ProjectBuilder build

automatically define _Macintosh_OSX if XCode build
